• There is a $2.00 processing fee for each school.

• Transcript requests should only be made AFTER you have added your schools on SCOIR and are ready for your documents to be sent to the college.

• Do not include any self-report colleges that we do not need to send information to.

• If you are applying to more than 5 schools, please complete another form.
